Construction injuries are not evenly spread across the hundreds of things that happen on a site. A short list of hazards accounts for most of the serious ones, which is good news: attention concentrated in a few places does most of the work.

1. Falls from height

Consistently the largest cause of serious injury and death in construction worldwide. It is also the hazard where controls are best understood and most often skipped for convenience.

What works, in order of preference: avoid the work at height entirely by assembling at ground level and lifting; then collective protection such as full edge protection, properly boarded scaffold and covered openings; then, only where neither is possible, personal fall arrest with a tested anchor point and someone trained to use it.

Openings and edges

Floor openings and unprotected leading edges cause falls after the exciting phase of the job has passed. Cover every opening as it is formed, fix the cover so it cannot be moved casually, and mark it. An uncovered stair void on a quiet Saturday has hurt more people than most exotic hazards.

2. Excavations

A cubic metre of soil weighs well over a tonne, and a collapse gives no warning. The controls are unglamorous: batter the sides back to a safe angle or shore them, keep spoil and plant well back from the edge, provide a proper ladder access, and inspect the excavation before each shift and after rain.

Rain is the trigger that catches people out. An excavation that stood safely for a week can fail after one heavy night. Inspection after rain is not a formality.

3. Lifting operations

Cranes and hoists concentrate enormous energy in one place. Most incidents come from one of four things: an overloaded or badly rigged lift, a load swung over people, equipment that was not inspected, or an operator and slinger who could not communicate.

The practical controls: a lift plan for anything non-routine, certified and inspected lifting gear, an exclusion zone that is enforced rather than announced, and one appointed signaller whom the operator takes instructions from. Nobody walks under a suspended load, ever, and that rule is worth enforcing loudly the first time it is broken.

4. Plant and vehicle movements

People struck by moving plant is a persistent cause of serious injury, most often when a machine reverses. Excavator operators and truck drivers have limited visibility and heavy machines take time to stop.

Separate people from plant wherever you can: dedicated pedestrian routes, physical barriers, one-way traffic circulation to reduce reversing, banksmen for reversing manoeuvres, and high-visibility clothing that is actually worn. On tight urban sites, a traffic management plan agreed before work starts prevents most of it.

5. Electricity

Two distinct dangers: overhead lines during lifting or tipping, and site power distribution in wet conditions.

For overhead lines, establish exclusion zones and goal posts before plant arrives, and treat every line as live. For site power, use proper distribution boards with residual current protection, keep leads out of water and off walkways, and remove damaged tools from use rather than promising to fix them later.

The controls that hold across all five

Beyond hazard-specific measures, three habits reduce every category:

  1. A short daily briefing. Five minutes on what is happening today and where the interfaces are. Most incidents happen where two trades meet.
  2. Inspection with authority behind it. Someone who walks the site daily and whose instruction to stop is respected.
  3. Near-miss reporting without blame. Near misses are free information about where the next injury will come from. Sites that punish reports simply stop hearing about them.

What good looks like

You can read a site in a few minutes. Are openings covered and edges protected? Is the excavation battered or shored, with a ladder in it? Is anyone standing under a load? Do pedestrians have a route that does not cross the plant route? Are leads out of the water?

Five questions, walked rather than filed. A site that answers them well is usually well run in every other respect too, because the same discipline that protects people also protects the programme.
